smoke filled room

smoke flows into the room when fire door is opened to add logs.
it is an insert and i have all sides sealed good. smoke does flow from the chimmy (which has a screen cap) but smoke comes out when you open door more than 1"1/2
any thoughts as to why??or how to fix??
